Terms and Conditions 

 

1. Services Offered

We provide mobile phlebotomy services, including but not limited to blood draws, specimen collection, and related laboratory testing at a location of your choice within New Jersey. Services are available by appointment only, and can be scheduled through our customer service team.

2. Client Responsibilities

  • Access to the Location: You agree to provide us with access to the designated location at the agreed-upon time, ensuring the area is safe, clean, and suitable for the performance of phlebotomy services.

  • Accurate Information: You must provide accurate and complete information regarding  any medications you are taking, and any other relevant details that may affect the phlebotomy process or the safety of the procedure.

  • Informed Consent: By requesting phlebotomy services, you confirm that you have provided informed consent for the procedure, which may include blood draws or other specimen collection.

3. Scheduling and Cancellations

  • Scheduling: All appointments for phlebotomy services must be scheduled in advance. You will receive confirmation of the scheduled time and location via email or phone.

  • Cancellations: You must notify us at least [24-48 hours] before the scheduled appointment if you wish to cancel or reschedule. Failure to do so may result in a cancellation fee.

4.  Payment Terms

  • Service Fees: The cost of phlebotomy services will be disclosed prior to the appointment, and payment is due at the time of service unless otherwise agreed. 

     

5. Health and Safety

  • Health Conditions: You agree to notify us of any health conditions, allergies, or recent surgeries that may impact the phlebotomy procedure. It is your responsibility to disclose any conditions that could affect the safety or efficacy of the procedure.

  • Emergency Situations: If a medical emergency arises during the procedure, we may contact emergency medical services. You will be responsible for any related costs.

  • Our phlebotomist are trained professionals who follow strict safety protocols to ensure a sterile and hygienic environment during blood collection procedures. 

6. Confidentiality and Privacy

  • Data Protection: We are committed to protecting your privacy and maintaining the confidentiality of your personal and medical information in accordance with applicable laws, including the Health Insurance Portability and Accountability Act (HIPAA).

 

7. Liability 

  • While every effort is made to minimize discomfort and risks associated with blood collection, clients acknowledge that there may be inherent risks. Our business and staff are not liable for any adverse reactions or complications that may arise during or after the procedure.

 

8. Minors

  • A legal guardian must accompany minors during blood collection  procedures.
